Silver struck medal with obverse showing female figure holding cornucopia and wreath; baled straw in background; inscription in exergue: "CERES." Reverse shows space for engraving at center surrounded by wreath of corn, fruit, and vegetables with engraved inscription: "To/Mrs./Theodore Allen/Premium/on Butter"; legend around border: "NEW YORK STATE AGRICULTURAL SOCIETY." Square burgundy red leather fitted case with burgundy red silk velvet lining.
