Bronze struck medal with obverse showing nude male bust facing left with legend running vertically at right of image: "JOHN HORN Jr." Reverse bears inscription within central roundel, surrounded by laurel wreath: "IN/ RECOGNITION/OF HIS/HEROIC EXPLOITS/IN/RESCUING MEN/WOMEN & CHILDREN/FROM DROWNING/IN/DETROIT RIVER"; legend within line border: "BY ACT OF CONGRESS JUNE 20th 1874."
